Taking the P3 form in Kenya involves a process primarily associated with police reports and medical examinations in cases of physical assault, road accidents, or other incidents requiring legal documentation. Here’s how to go about it:

Step 1: Report the Incident

  • Visit the Police Station: Immediately after the incident, go to the nearest police station to report the case. This is necessary for obtaining a police abstract, which is a prerequisite for the P3 form.

Step 2: Obtain the P3 Form

  • Request the Form: At the police station, request a P3 form. This form is provided free of charge and is used to document the nature and extent of injuries for legal purposes.
  • Police Endorsement: The police officer handling your case will fill out the first part of the P3 form, which includes details of the incident and the official police remarks.

Step 3: Undergo Medical Examination

  • Visit a Government Doctor: Take the P3 form to a government doctor or any other doctor authorized by the government to conduct P3 examinations. Private doctors can also fill out the form, but they must be licensed and recognized by the government.
  • Medical Examination: The doctor will examine you to assess the injuries or harm sustained during the incident. The findings will be recorded on the P3 form.

Step 4: Return the Form to the Police

  • Doctor’s Report: After the medical examination, the doctor will complete their section of the P3 form, sign it, and stamp it.
