Here’s a step-by-step guide to acquiring and properly processing a P3 form in Kenya:


Step 1: Report the Incident to the Police

  • Go to the Nearest Police Station
    Immediately after the incident, report the matter at your local police station. A police officer will document the case and issue a police abstract, which is required before accessing a P3 form.

Step 2: Obtain the P3 Form

  • Request the P3 Form
    Ask the officer handling your case for a P3 form. It is issued free of charge and serves as an official record of injuries sustained.
  • Initial Police Endorsement
    The officer will fill in Part I of the P3 form. This section contains incident details such as the date, time, nature of the case, and the officer’s remarks.

Step 3: Undergo a Medical Examination

  • Visit an Authorized Medical Officer
    Take the partially filled P3 form to a government doctor or licensed private medical practitioner. The doctor must be authorized to conduct P3 assessments.
  • Injury Assessment
    The doctor will examine the injuries and fill out Part II of the P3 form, documenting the nature and severity of harm.

Step 4: Return the Completed P3 Form to Police

  • Doctor’s Endorsement
    Once the medical report section is filled, the doctor will sign and stamp the form, confirming the findings.
  • Submit the Form Back to the Police
    Return the fully completed P3 form to the police station where you initially filed the report. It becomes part of the case file for further investigation or prosecution.

  • Keep a Copy
    Retain a copy of the P3 form for your records. It is often needed in court or during compensation and insurance claim processes.
