How to Request “To Whom it May Concern” Certificate for Qataris (To Specify the Type of Disability). This service, provided by the Community Care Department at the Ministry of Social Development and Family (MSDF), allows Qataris to obtain the certificate, in which the type of disability is specified, to make it easy for them to receive most government services.
Offline Instructions
- Download and fill out the application form.
- Attach copies of the following documents:
- New medical report which validity must not exceed (6 months), issued by Hamad Medical Corporation
- Applicant’s ID card
- Hospital appointment abroad (in case treatment will be received abroad only)
- Submit the application and all the required documents.
