How to Apply for a Probable Age Certificate in Sri Lanka

Eligibility to Apply for a Probable Age Certificate in Sri Lanka
Certificate of Probable Age is issued to a:
- resident child of a children’s home approved by the government.
- child under the guardianship of persons other than a government approved children’s home.
Required Documents to Apply for a Probable Age Certificate in Sri Lanka
Documents to be submitted by a resident child of a children’s home approved by the government:
- Affidavit of the matron of the children’s home
- Medical certificate issued by a government medical officer determining the age of the child *Other documents containing information of the child (if any)
Documents to be submitted by a child under the guardianship of persons other than a government approved children’s home:
- Affidavit of the guardian
- Proof of guardianship :
- Grama Niladhari certificate or
- Certificate of the probation and Childcare Service Officer
- Certificate from the Social Services Department
- Certificate issued by the Inspector of Police
- Medical certificate issued by a government medical officer determining the age of the child *Health Development Record
- Copy of the school admission register if the child attends school
